· Entry fee includes golf, cart, boxed lunch, range balls & prizes
· Split tee times—9:30-10:30 a.m. — Arrive 20 minutes prior
· Tee times will be emailed on Thursday, November 4
· Winners will be announced via email and prize money mailed
· Signed waiver required to participate—return with your mailed entry form if you do not have one on file
